All too often the underlying ADHD symptomatology lies hidden beneath subsequently developed disorders (such as obsessive rumination disorder, anxieties, panic attacks and addictions). Patients and friends have been so kind, embellishing this book with their contributions, sketches, pictures and poems. diagnostic interview for adhd in adults diva 2.0: LA INTELIGENCIA EMOCIONAL EN ADULTOS CON TRASTORNO POR DÉFICIT DE ATENCIÓN CON HIPERACTIVIDAD Y LA RELACION CON SU CALIDAD DE VIDA Doctora Rosa Vera García, 2016-06-11 El Trastorno por Déficit de Atención con Hiperactividad (TDAH), uno de los más comunes trastornos en la infancia, suele permanecer a lo largo de la vida de la persona, causando una importante disfuncionalidad en diferentes áreas vitales y afectando a su calidad de vida (QoL). Diferentes evidencias indican que el TDAH podría permanecer en la edad adulta en, al menos, un 50% de los casos. A pesar de la larga trayectoria de estudios e investigaciones, aún no se conocen con exactitud los precursores biológicos y no han quedado definitivamente clarificadas las causas genéticas, neurológicas, conductuales y psicosociales. Aunque el TDAH cuenta con más de un siglo de investigaciones, no es hasta la década de los 60 del siglo pasado cuando se comienza a mostrar evidencia de que el trastorno presenta tendencia a permanecer en la edad adulta. Y es a partir de los años 90, cuando los estudios validan las diferencias entre adultos con y sin TDAH. Empieza a desvelarse su trasfondo neurobiológico y a realizarse estudios longitudinales en población infantil, comenzando a demostrarse la cronicidad del trastorno. Esta investigación tan tardía ha podido dar lugar a que se haya producido un infradiagnóstico en el adulto, mostrando cifras por las que solo un 25% habría sido diagnosticado. Personas con TDAH suelen desarrollar otras patologías psiquiátricas, lo que supondría una importante complicación tanto para el diagnóstico como para la evolución del trastorno. Las patologías psiquiátricas comórbidas al TDAH más frecuentes suelen ser el trastorno de ansiedad, trastorno depresivo mayor, trastorno bipolar, trastorno negativista desafiante, trastorno disocial, dependencia o abuso de sustancias legales o ilegales, etc. Se estima que, entre un 25-50% de los pacientes adultos con TDAH, presentan patología psiquiátrica comórbida. En lo referente a la epidemiología del trastorno en la edad adulta, ésta es aún bastante controvertida, pudiendo variar en función del contexto geográfico, los criterios diagnósticos, así como la diversidad de instrumentos de evaluación y puntos de corte. Se muestran cifras que oscilan entre el 4,5% para Estados Unidos, Oriente Medio y América del Sur, y un 1-3% para Europa. En población española, la prevalencia apenas llega al 1,2%. En cuanto a las diferencias de género, existe una mayor prevalencia de niños frente a niñas, que se reduciría en la edad adulta, aunque suele permanecer a favor de los hombres. No existe un claro consenso respecto a la etiología del trastorno, y es el modelo multifactorial el que podría actualmente ser la mejor explicación. Encontramos factores genéticos, neuroquímicos, alteraciones cerebrales a nivel estructural y funcional y diversos factores ambientales. La reciente edición DSM-5 ha adaptado los criterios diagnósticos del adulto, introduciendo algunas modificaciones para su mejor adaptación a esta etapa. Según esta edición, la manifestación clínica del TDAH en el adulto se va a caracterizar principalmente por sintomatología de inatención e impulsividad, quedando atenuada la hiperactividad. En la edad adulta, el trastorno suele seguir provocando una importante disfuncionalidad en forma de peores rendimientos en los ámbitos personales y profesionales, con una tendencia a presentar un estilo de vida “caótico”, suponiendo un importante deterioro de su QoL. Asimismo, personas con TDAH suelen mostrar bajos niveles de eficacia emocional junto a una mayor impulsividad y falta de habilidades sociales. Esta circunstancia elevaría la susceptibilidad de estar implicadas en diversas formas de comportamiento antisocial, así como mayores niveles de estrés y dificultades emocionales en diferentes ámbitos, laboral, escolar, relaciones personales, etc. Tradicionalmente, la inteligencia cognitiva ha sido prácticamente la única variable que se reconocía como predictora del éxito de la persona, pero desde la aparición del constructo de inteligencia emocional (IIEE) se ha venido demostrando a través de la investigación que la parte afectiva del ser humano no debía quedar relegada. Las emociones influirían en la cognición, por lo que podrían contribuir al mejor ajuste vital. En nuestra investigación, la IIEE ha sido entendida desde el Modelo de Habilidades de las cuatro ramas, de Mayer y Salovey. Este modelo concibe la IIEE como un conjunto de habilidades cognitivas referidas a las emociones, que no tiene relación con el factor personalidad y presenta una baja o nula relación con el factor CI. Según estos autores, personas con mejores habilidades en la percepción de emociones, con mejor uso de éstas para facilitar el pensamiento, mejor comprensión y manejo, de las propias así como de las ajenas, las personas gestionan sus vidas de forma más adaptativa. La IIEE, bajo este modelo, es considerada un tipo de inteligencia, habiendo sido demostrada su independencia con respecto al CI. Esto posibilitaría su desarrollo y mejora, y que pudiera ser incluida en los tratamientos multimodales de intervención terapéutica para el TDAH. Un tipo de inteligencia genuina, que se fundamenta en el uso adaptativo de las emociones y su aplicación al pensamiento, pasando desde los procesos más básicos (percepción emocional) hasta los de mayor complejidad (comprensión y regulación emocional). Las habilidades emocionales y las cognitivas serían factores clave para lograr un mayor bienestar de la persona. El concepto de QoL, aunque falto de consenso en su definición, incorpora aspectos económicos, médicos y sociales como satisfacción con la vida, bienestar subjetivo, estado de salud física y mental, felicidad, funcionalidad, etc. El TDAH suele conllevar un impacto negativo significativo en una gran variedad de dimensiones del funcionamiento diario, teniendo como consecuencia un menor nivel de valoración de su QoL. Hoy en día, la evaluación de la QoL de sujetos con TDAH, en nuestra opinión, es aún escasa, y también lo es una profunda comprensión de los factores que sobre ella influyen. |
