MILLIS In 1966 the Citizens Commission on Graduate Medical Education observed that the explosive growth in biomedical science and the consequent increase in medical skill and technology of the twentieth century had made it possible for physicians to respond to the episodes of illness of patients with an ever-increasing effectiveness, but that the increase in knowledge and technology had forced most physicians to concentrate upon a disease entity, an organ or organ system, or a particular mode of diagnosis or therapy. As a result there had been a growing lack of continuing and comprehensive patient care. The Commission expressed the opinion that Now, in order to bring medicine's enhanced diagnostic and therapeutic powers fully to the benefit of society, it is necessary to have many physicians who can put medicine together again. ! The Commission proceeded to recommend the education and training of sub stantial numbers of Primary Physicians who would, by assuming primary responsi bility for the patient's welfare in sickness and in health, provide continuing and comprehensive health care to the citizens of the United States. In 1978 it is clear that the recommendation has been accepted by the public, the medical profession, and medical education. There has been a vigorous response in the development of family medicine and in the fields of internal medicine, pediatrics, and obstetrics. As undergraduates in teaching hospitals future physicians are taught to think in clear and absolute black and white terms. Diagnoses in teaching hospitals all are based on supportive positive findings of in vestigations. Treatment follows logically on precise diagnosis. When patients die the causes of death are confirmed at autopsy. How very different is real life in clinical practice, and particularly in family medicine. By the very nature of the common conditions that present diagnoses tend to be imprecise and based on clinical assessment and interpretation. Much of the management and treatment of patients is based on opinions of individual physicians based on their personal expenences. Because of the relative professional isolation offamily physicians within their own practices, not unexpectedly divergent views and opinions are formed. There is nothing wrong in such divergencies because there are no clear absolute black and white decisions. General family practice functions in grey areas of medicine where it is possible and quite correct to hold polarized distinct opinions. The essence of good care must be eternal flexibility and readiness to change long-held cherished opinions. To demonstrate that with many issues in family medicine it is possible to have more than one view I selected 10 clinical and II non -clinical topics and invited colleagues and fellow-practitioners to enter into a debate-in-print. |

This class of drugs, known as oral phosphodiesterase inhibitors (PDE-type 5), is highly effective in the treatment of ED. Since its introduction there has been a much greater awa- ness of ED, its comorbidities, and its effects on the quality of life. In 1997, while preparing to address the Endocrine Society on the occasion of the 92nd American Urological Association meeting, I first looked at the p- clinical studies of sildenafil. I thought “this will change everything” and it clearly has—changing practice patterns in sexual medicine, and the at- tudes of patients, potential patients, and their partners. Two new PDE-type ® ® 5 inhibitors, tadalafil (Cialis ) and vardenafil (Levitra ), were first approved by the European Committee for Proprietary Medicinal Products and subsequently by the Food and Drug Administration in 2003 and 2004. |
