The clinical ranges for osteoporosis, osteopenia, and normal bone density are presented. Osteoporosis affects 2 percent of men and 10 percent of women over the age of 50 in the United States. In addition, 49 percent of older women and 30 percent of older men in the United States have low bone density or osteopenia. Osteoporosis is a significant public health problem that leads to increased bone fragility and greater fracture risk, especially of the wrist, hip, and spine. In an epidemiological study conducted in Switzerland, 50 percent of all fractures in women and 24 percent in men were considered osteoporotic. In the United States an estimated 1.5 million yearly osteoporotic fractures result in more than 500,000 hospitalizations, 800,000 emergency room visits, 2.6 million physician office visits, and 180,000 nursing home placements. Hip fractures, in particular, are associated with an increased risk of death. Fractures can also cause pain, height loss, and functional disability, as well as complications such as pressure sores and pneumonia. By 2020, approximately half of all older Americans will be at risk for fractures from osteoporosis or osteopenia. The U.S. Preventive Services Task Force recommends active screening for osteoporosis and early intervention to prevent bone fractures. Current clinical guidelines recommend dietary and pharmacological interventions to treat osteoporosis and prevent bone fractures. An increase of 1 standard deviation in bone mineral density in women would prevent 33 percent of hip fractures and 77 percent of vertebral fractures. Despite proven effectiveness, these treatments may have low rates of long-term adherence. Pharmacological interventions can result in adverse outcomes, commonly minimal trauma atypical fractures, esophageal irritation, renal toxicity, and osteonecrosis of the jaw. Additionally, requirements of pharmacological interventions may be burdensome for patients. How vibration therapy increases bone density is not well understood. One hypothesis suggests that vibration signals transmit and amplify into bone tissue, directly activating mechanosensors in bone cells. Animal studies have demonstrated that vibration increases the anabolic (bone building) activity of bone tissue and increases bone density. Another hypothesis suggests that whole-body vibration, like other weight-bearing exercise, improves muscle strength and power by increasing neuromuscular activation. Human studies on healthy volunteers examined adaptive muscle strength and performance after vibration therapy and found its effects to be similar to those of short-term resistance exercise. Several studies have shown whole-body vibration therapy to improve muscle and bone circulation, increasing the supply of nutrients needed to build bones. This technical brief describes the state of the science and summarizes the key issues related to the use of whole-body vibration therapy to improve bone density for the prevention and treatment of osteoporosis, including modalities, standards, relevant patient populations, outcomes measured, and implications for future research. This report's scope is confined to whole-body vibration platforms designed and marketed for prevention and treatment of osteoporosis; our review excludes exercise equipment with vibrating platforms intended for use in physical fitness or athletic regimens. |

Whole-body vibration therapy has been proposed as an alternative or adjunctive intervention, but its role in preventing and treating osteoporosis, and the populations in which it has been studied, is unclear. PURPOSE: To provide an overview of the key issues and evidence map related to the use of whole-body vibration therapy for the prevention and treatment of osteoporosis. METHODS: A review of the published and grey literature and interviews with Key Informants. FINDINGS: Very little scientific evidence evaluates the benefits and harms of whole-body vibration therapy for the prevention and treatment of osteoporosis; only 12 studies met the inclusion criteria for our review. A number of questions regarding the optimal population for treatment, optimal treatment protocol, key outcome measures, and whether whole-body vibration therapy is an adjunctive or distinctive therapy emerged from the published literature and key informant discussions. Reviewed studies offered little information on potential harms. However, safety concerns emerged from key informant discussions, including unknown long-term harms from the use of whole-body vibration therapy, and the potential inability of consumers to clearly distinguish low-intensity platforms intended for osteoporosis therapy from platforms intended for high intensity exercise. Claims about whole-body vibration therapy for the prevention and treatment of osteoporosis cannot be made without further research. |

During childhood and adolescence bone formation is dominant; bone length and girth increase with age ending at early adulthood when peak bone mass is attained. Males generally exhibit a longer growth period resulting in bones of greater size and overall strength. In males after the age of 20 bone resorbtion becomes predominant and bone mineral content declines about 4% per decade. Females tend to maintain peak mineral content until menopause at which time it declines about 15% per decade. Osteoporosis is a disease characterized by low bone mass and structural deterioration of bone tissue leading to bone fragility and an increased susceptibility to fractures especially of the hip spine and wrist. Osteoporosis occurs primarily as a result of normal ageing but can arise as a result of impaired development of peak bone mass (e.g. due to delayed puberty or undernutrition) or excessive bone loss during adulthood (e.g. due to estrogen deficiency in women undernutrition or corticosteriod use). Osteoporosis-induced fractures cause a great burden to society. Hip fractures are the most serious as they nearly always result in hospitalization are fatal about 20% of the time and produce permanent disability about half the time. Fracture rates increase rapidly with age and the lifetime risk of fracture in 50 year-old women is about 40% similar to that for coronary heart disease. In 1990 there were 1.7 million hip fractures alone worldwide; with changes in population demographics this figure is expected to rise to 6 million by 2050. To help describe the nature and consequences of osteoporosis as well as strategies for its prevention and management a WHO Scientific Group meeting of international experts was held in Geneva which resulted in this technical report. This monograph describes in detail normal bone development and the causes and risk factors for developing osteoporosis. The burden of osteoporosis is characterized in terms of mortality morbidity and economic costs. Methods for its prevention and treatment are discussed in detail for both pharmacological and non-pharmacological approaches. For each approach the strength of the scientific evidence is listed. The report also provides cost-analysis information for potential interventions and discusses important aspects of developing national policies to deal with osteoporosis. Recommendations are made to the general population care providers health administrators and researchers. Lastly national organizations and support groups are listed by country. |
