dr douglas schechter pain management: Relieving Pain in America Institute of Medicine, Board on Health Sciences Policy, Committee on Advancing Pain Research, Care, and Education, 2011-10-26 Chronic pain costs the nation up to $635 billion each year in medical treatment and lost productivity. The 2010 Patient Protection and Affordable Care Act required the Department of Health and Human Services (HHS) to enlist the Institute of Medicine (IOM) in examining pain as a public health problem. In this report, the IOM offers a blueprint for action in transforming prevention, care, education, and research, with the goal of providing relief for people with pain in America. To reach the vast multitude of people with various types of pain, the nation must adopt a population-level prevention and management strategy. The IOM recommends that HHS develop a comprehensive plan with specific goals, actions, and timeframes. Better data are needed to help shape efforts, especially on the groups of people currently underdiagnosed and undertreated, and the IOM encourages federal and state agencies and private organizations to accelerate the collection of data on pain incidence, prevalence, and treatments. Because pain varies from patient to patient, healthcare providers should increasingly aim at tailoring pain care to each person's experience, and self-management of pain should be promoted. In addition, because there are major gaps in knowledge about pain across health care and society alike, the IOM recommends that federal agencies and other stakeholders redesign education programs to bridge these gaps. Pain is a major driver for visits to physicians, a major reason for taking medications, a major cause of disability, and a key factor in quality of life and productivity. Given the burden of pain in human lives, dollars, and social consequences, relieving pain should be a national priority.

dr douglas schechter pain management: Psychophysiologic Disorders Howard Schubiner, Mags Clark-Smith, Allan Abbass, David Clarke, 2019-11-13 Did you know that one in six adults and 30-40% of primary care patients suffer from medically unexplained symptoms, chronic functional syndromes or psychosocial factors linked to chronic pain? Collectively these are known as Psychophysiologic Disorders or PPD. A trauma-informed, evidence-based approach to diagnosis and treatment can transform these patients from among the most frustrating to the most rewarding and give them a far better chance for a full recovery. As one family physician who learned these concepts said: It put the joy back into my practice. From this innovative book, medical and mental health professionals will learn to relieve (not just manage) physical symptoms by assessing for and treating current life stresses, past traumas, suppressed emotions and the prolonged impact of adverse childhood experiences (ACEs). The sixteen authors from five countries average 20 years of experience in the fields of Adolescent Medicine, Family Medicine, Gastroenterology, Health Journalism, Integrative Medicine, Internal Medicine, Movement Therapy, Neuroscience, Orthopedic Spine Surgery, Pain Medicine, Physiotherapy, Psychiatry, Psychoanalysis, Psychology, Psychotherapy, and Sports Medicine. From this wide range of backgrounds, the authors reached consensus on a core set of practices that were a revelation for them and their patients. These concepts are practical and can readily be implemented by any healthcare professional. In addition to the editors, chapter authors include James Alexander PhD, Mariclare Dasigenis LCSW, David Hanscom MD, Ian Kleckner PhD MPH, Mark Lumley PhD, Daniel Lyman LCSW MPA, Meghan Maguire, Georgie Oldfield MCSP, David Schechter MD, Eric Sherman PsyD, John Stracks MD, and Joel Town DClinPsy.

This area of research is not limited to very young and old patientsâ€sleep disorders reach across all ages and ethnicities. Sleep Disorders and Sleep Deprivation presents a structured analysis that explores the following: Improving awareness among the general public and health care professionals. Increasing investment in interdisciplinary somnology and sleep medicine research training and mentoring activities. Validating and developing new and existing technologies for diagnosis and treatment. This book will be of interest to those looking to learn more about the enormous public health burden of sleep disorders and sleep deprivation and the strikingly limited capacity of the health care enterprise to identify and treat the majority of individuals suffering from sleep problems.
dr douglas schechter pain management: Chronic Graft Versus Host Disease Georgia B. Vogelsang, Steven Z. Pavletic, 2009-04-20 Chronic graft versus host disease (GVHD) is the most common complication of allogenic bone marrow transplantation. Because of the protracted clinical course of chronic GVHD, transplant centers and hematology/oncology offices are inadequately equipped to manage these immuno-incompetent patients with a multi-system disorder. Practitioners need to be able to recognize and effectively manage chronic GVHD as a late effect of more than half of allogenic transplantations. The text is oriented for the clinician, with chapters covering staging, organ site and system-specific manifestations, treatment options, and supportive care. Drs Georgia B. Vogelsang and Steven Z. Pavletic have been pioneers in the recognition of the multi-organ complexity of this disease and have gathered the input of a variety of subspecialist physicians for this book. This book fills the gap in practical literature on chronic GVHD, providing a comprehensive, up-to-date, and clinically relevant resource for anyone who deals with cancer patients post-transplant.

dr douglas schechter pain management: Improving Palliative Care for Cancer National Research Council, Institute of Medicine, National Cancer Policy Board, 2001-10-19 In our society's aggressive pursuit of cures for cancer, we have neglected symptom control and comfort care. Less than one percent of the National Cancer Institute's budget is spent on any aspect of palliative care research or education, despite the half million people who die of cancer each year and the larger number living with cancer and its symptoms. Improving Palliative Care for Cancer examines the barriersâ€scientific, policy, and socialâ€that keep those in need from getting good palliative care. It goes on to recommend public- and private-sector actions that would lead to the development of more effective palliative interventions; better information about currently used interventions; and greater knowledge about, and access to, palliative care for all those with cancer who would benefit from it.

dr douglas schechter pain management: Redefining Federalism Douglas T. Kendall, 2004 If federalism is about protecting the states, why not listen to them? In the last decade, the Supreme Court has reworked significant areas of constitutional law with the professed purpose of protecting the dignity and authority of the states, while frequently disregarding the states'' views as to what federalism is all about. The Court, according to the states, is protecting federalism too much and too little. Too much, in striking down federal law where even the states recognize that a federal role is necessary to address a national problem. Too little, in inappropriately limiting state experimentation. By listening more carefully to the States, the Supreme Court could transform its federalism jurisprudence from a source of criticism and polarization to a doctrine that should win broad support from across the political spectrum. In this important book, six distinguished authors redefine federalism and reaffirm Justice Louis Brandeis's vision of states and localities as the laboratories of democracy.

dr douglas schechter pain management: Temporomandibular Disorders National Academies of Sciences, Engineering, and Medicine, Health and Medicine Division, Board on Health Care Services, Board on Health Sciences Policy, Committee on Temporomandibular Disorders (TMDs): From Research Discoveries to Clinical Treatment, 2020-07-01 Temporomandibular disorders (TMDs), are a set of more than 30 health disorders associated with both the temporomandibular joints and the muscles and tissues of the jaw. TMDs have a range of causes and often co-occur with a number of overlapping medical conditions, including headaches, fibromyalgia, back pain and irritable bowel syndrome. TMDs can be transient or long-lasting and may be associated with problems that range from an occasional click of the jaw to severe chronic pain involving the entire orofacial region. Everyday activities, including eating and talking, are often difficult for people with TMDs, and many of them suffer with severe chronic pain due to this condition. Common social activities that most people take for granted, such as smiling, laughing, and kissing, can become unbearable. This dysfunction and pain, and its associated suffering, take a terrible toll on affected individuals, their families, and their friends. Individuals with TMDs often feel stigmatized and invalidated in their experiences by their family, friends, and, often, the health care community. Misjudgments and a failure to understand the nature and depths of TMDs can have severe consequences - more pain and more suffering - for individuals, their families and our society. Temporomandibular Disorders: Priorities for Research and Care calls on a number of stakeholders - across medicine, dentistry, and other fields - to improve the health and well-being of individuals with a TMD. This report addresses the current state of knowledge regarding TMD research, education and training, safety and efficacy of clinical treatments of TMDs, and burden and costs associated with TMDs. The recommendations of Temporomandibular Disorders focus on the actions that many organizations and agencies should take to improve TMD research and care and improve the overall health and well-being of individuals with a TMD.
