emt-a scope of practice: Crisis Standards of Care Institute of Medicine, Board on Health Sciences Policy, Committee on Guidance for Establishing Standards of Care for Use in Disaster Situations, 2012-08-26 Catastrophic disasters occurring in 2011 in the United States and worldwide-from the tornado in Joplin, Missouri, to the earthquake and tsunami in Japan, to the earthquake in New Zealand-have demonstrated that even prepared communities can be overwhelmed. In 2009, at the height of the influenza A (H1N1) pandemic, the Assistant Secretary for Preparedness and Response at the Department of Health and Human Services, along with the Department of Veterans Affairs and the National Highway Traffic Safety Administration, asked the Institute of Medicine (IOM) to convene a committee of experts to develop national guidance for use by state and local public health officials and health-sector agencies and institutions in establishing and implementing standards of care that should apply in disaster situations-both naturally occurring and man-made-under conditions of scarce resources. Building on the work of phase one (which is described in IOM's 2009 letter report, Guidance for Establishing Crisis Standards of Care for Use in Disaster Situations), the committee developed detailed templates enumerating the functions and tasks of the key stakeholder groups involved in crisis standards of care (CSC) planning, implementation, and public engagement-state and local governments, emergency medical services (EMS), hospitals and acute care facilities, and out-of-hospital and alternate care systems. Crisis Standards of Care provides a framework for a systems approach to the development and implementation of CSC plans, and addresses the legal issues and the ethical, palliative care, and mental health issues that agencies and organizations at each level of a disaster response should address. Please note: this report is not intended to be a detailed guide to emergency preparedness or disaster response. What is described in this report is an extrapolation of existing incident management practices and principles. Crisis Standards of Care is a seven-volume set: Volume 1 provides an overview; Volume 2 pertains to state and local governments; Volume 3 pertains to emergency medical services; Volume 4 pertains to hospitals and acute care facilities; Volume 5 pertains to out-of-hospital care and alternate care systems; Volume 6 contains a public engagement toolkit; and Volume 7 contains appendixes with additional resources.
emt-a scope of practice: Emergency Medical Services Institute of Medicine, Board on Health Care Services, Committee on the Future of Emergency Care in the United States Health System, 2007-06-03 Emergency Medical Services (EMS) is a critical component of our nation's emergency and trauma care system, providing response and medical transport to millions of sick and injured Americans each year. At its best, EMS is a crucial link to survival in the chain of care, but within the last several years, complex problems facing the emergency care system have emerged. Press coverage has highlighted instances of slow EMS response times, ambulance diversions, trauma center closures, and ground and air medical crashes. This heightened public awareness of problems that have been building over time has underscored the need for a review of the U.S. emergency care system. Emergency Medical Services provides the first comprehensive study on this topic. This new book examines the operational structure of EMS by presenting an in-depth analysis of the current organization, delivery, and financing of these types of services and systems. By addressing its strengths, limitations, and future challenges this book draws upon a range of concerns: • The evolving role of EMS as an integral component of the overall health care system. • EMS system planning, preparedness, and coordination at the federal, state, and local levels. • EMS funding and infrastructure investments. • EMS workforce trends and professional education. • EMS research priorities and funding. Emergency Medical Services is one of three books in the Future of Emergency Care series. This book will be of particular interest to emergency care providers, professional organizations, and policy makers looking to address the deficiencies in emergency care systems.
emt-a scope of practice: Emergency Medical Services Jane H. Brice, Theodore R. Delbridge, J. Brent Myers, 2021-08-12 The two-volume Emergency Medical Services: Clinical Practice and Systems Oversight delivers a thorough foundation upon which to succeed as an EMS medical director and prepare for the NAEMSP National EMS Medical Directors Course and Practicum. Focusing on EMS in the 'real world', the book offers specific management tools that will be useful in the reader's own local EMS system and provides contextual understanding of how EMS functions within the broader emergency care system at a state, local, and national level. The two volumes offer the core knowledge trainees will need to successfully complete their training and begin their career as EMS physicians, regardless of the EMS systems in use in their areas. A companion website rounds out the book's offerings with audio and video clips of EMS best practice in action. Readers will also benefit from the inclusion of: A thorough introduction to the history of EMS An exploration of EMS airway management, including procedures and challenges, as well as how to manage ventilation, oxygenation, and breathing in patients, including cases of respiratory distress Practical discussions of medical problems, including the challenges posed by the undifferentiated patient, altered mental status, cardiac arrest and dysrhythmias, seizures, stroke, and allergic reactions An examination of EMS systems, structure, and leadership
emt-a scope of practice: Strategies to Improve Cardiac Arrest Survival Institute of Medicine, Board on Health Sciences Policy, Committee on the Treatment of Cardiac Arrest: Current Status and Future Directions, 2015-09-29 Cardiac arrest can strike a seemingly healthy individual of any age, race, ethnicity, or gender at any time in any location, often without warning. Cardiac arrest is the third leading cause of death in the United States, following cancer and heart disease. Four out of five cardiac arrests occur in the home, and more than 90 percent of individuals with cardiac arrest die before reaching the hospital. First and foremost, cardiac arrest treatment is a community issue - local resources and personnel must provide appropriate, high-quality care to save the life of a community member. Time between onset of arrest and provision of care is fundamental, and shortening this time is one of the best ways to reduce the risk of death and disability from cardiac arrest. Specific actions can be implemented now to decrease this time, and recent advances in science could lead to new discoveries in the causes of, and treatments for, cardiac arrest. However, specific barriers must first be addressed. Strategies to Improve Cardiac Arrest Survival examines the complete system of response to cardiac arrest in the United States and identifies opportunities within existing and new treatments, strategies, and research that promise to improve the survival and recovery of patients. The recommendations of Strategies to Improve Cardiac Arrest Survival provide high-priority actions to advance the field as a whole. This report will help citizens, government agencies, and private industry to improve health outcomes from sudden cardiac arrest across the United States.

emt-a scope of practice: Wild Rescues Kevin Grange, 2021-04-06 Kevin Grange details nearly everything that possibly could go wrong in a national park and yet still manages to make you more excited than ever to hit the trail. —Conor Knighton, New York Times bestselling author of Leave Only Footprints: My Acadia-to-Zion Journey Through Every National Park Wild Rescues is a fast-paced, firsthand glimpse into the exciting lives of paramedics who work with the National Park Service: a unique brand of park rangers who respond to medical and traumatic emergencies in some of the most isolated and rugged parts of America. In 2014, Kevin Grange left his job as a paramedic in Los Angeles to work in a response area with 2.2 million acres: Yellowstone National Park. Seeking a break from city life and urban EMS, he wanted to experience pure nature, fulfill his dream of working for the National Park Service, and take a crash-course in wilderness medicine. Grange's epic journey took him to Yellowstone, Yosemite, and Grand Teton National Parks where, among other calls, he battled to save the lives of a heart attack victim at Old Faithful, a hiker who'd fractured his skull below Yosemite Falls, and a snowmobiler who launched into a deep gorge in the shadow of the jagged Tetons. Grange was initially overwhelmed—and out of his element—providing patient care in an extreme environment with limited resources and a two-hour drive to the nearest hospital. But he came to enjoy the challenges and steep learning curve of wilderness medicine. Between calls, Grange reflects upon the democratic ideal of the National Park mission, the beauty of the land, and the many threats facing it. With visitation rising, budgets shrinking, and people loving our parks to death, he realized that—along with the health of his patients—he was also fighting for the life of America's Best Idea.

emt-a scope of practice: Lights and Sirens Kevin Grange, 2015-06-02 A true account of going through UCLA’s famed Daniel Freeman Paramedic Program—and practicing emergency medicine on the streets of Los Angeles. Nine months of tying tourniquets and pushing new medications, of IVs, chest compressions, and defibrillator shocks—that was Kevin Grange’s initiation into emergency medicine when, at age thirty-six, he enrolled in the “Harvard of paramedic schools”: UCLA’s Daniel Freeman Paramedic Program, long considered one of the best and most intense paramedic training programs in the world. Few jobs can match the stress, trauma, and drama that a paramedic calls a typical day at the office, and few educational settings can match the pressure and competitiveness of paramedic school. Blending months of classroom instruction with ER rotations and a grueling field internship with the Los Angeles Fire Department, UCLA’s paramedic program is like a mix of boot camp and med school. It would turn out to be the hardest thing Grange had ever done—but also the most transformational and inspiring. An in-depth look at the trials and tragedies that paramedic students experience daily, Lights and Sirens is ultimately about the best part of humanity—people working together to help save a human life.

This new book considers the multiple aspects to the emergency care system in the United States by exploring its strengths, limitations, and future challenges. The wide range of issues covered includes: • The role and impact of the emergency department within the larger hospital and health care system. • Patient flow and information technology. • Workforce issues across multiple disciplines. • Patient safety and the quality and efficiency of emergency care services. • Basic, clinical, and health services research relevant to emergency care. • Special challenges of emergency care in rural settings. Ford, RN, PNP, EdD Dean Emeritus, University of Rochester School of Nursing Founder of the National Nurse Practitioner Program Member, National Women’s Hall of Fame Featuring the most current, valid, and reliable evidence-based content available, this three-time AJN winner once again presents an unparalleled resource for disaster and emergency preparedness. Disasters lay heavy burdens upon healthcare systems that stretch all levels of society. While natural and man-made disasters are not new, the global nature, rate, type, and totality of their impact has only increased. The fourth edition of this foundational text uniquely addresses the rapid changes in these crises and analyzes the latest attempts to provide timely, multidisciplinary healthcare. Nurses consistently comprise seventy or more percent of responders to local, national, regional, and global crises, and represent a potentially untapped resource to achieve surge capacity goals and optimize population health outcomes. Considering recent world events and increasing geopolitical tensions, Disaster Nursing and Emergency Preparedness, Fourth Edition now features expanded content on the ubiquitous threat of terrorism, potential detonation of thermonuclear weapons, emerging and reemerging infectious diseases, and increasing frequency and intensity of natural disasters from climate change. This text promotes competency-based expert nursing care during disasters and positive health outcomes for small and large populations consistent with the Federal Disaster Response Framework. Whenever possible, content is mapped to published core competencies for preparing health professions’ students for response to terrorism, disaster events, and public health emergencies. The fourth edition retains the clearly organized format in each chapter that includes an overview, learning objectives, a summary, and case study with reflective questions. New to the Fourth Edition: Addresses the most recent landmark agreements Sendai Framework for Disaster Risk Reduction 2015–2030, the United Nations’ Sustainable Development Goals, and the Paris Climate Agreement Covers public health emergencies involving community violence and civil unrest Expands coverage of planning for and accommodating high-risk, high-vulnerability populations Aligns with the U.S. National Health Security Strategy and the National Planning Frameworks Provides focused content on medical countermeasures Includes an improved instructor package with guide, PowerPoint slides, and case studies with questions for reflection in every chapter
